Roles & Responsibilities:

Perform consultations based on the results of the biometric screenings performed:

Blood pressure, BMI, and height and weight measurements

Cholesterol and Glucose screenings
-
Osteoporosis screenings
-

Perform the following screenings during the consultation:

Mild Cognitive Impairment screeding
-
PHQ-2 screeding
-
Medication adherence discussion
-
Smoking cessation
-
-
Share a list of local providers available on iPad
Qualifications:
-
Certified/Licensed Nurse Practitioner, Physician Assistant or Registered Nurse

Strong interpersonal and communication skills
-
Ability to work in a fast-paced environment with attention to detail

Familiarity with medical equipment and technology

Bilingual in Spanish
