The Licensed Practical Nurse (LPN) will perform health assessment and care coordination services to children in foster care. The LPN will provide assessment services to children in foster care in the support of pediatric care provided by our pediatricians and will also be responsible for the care coordination services. The individual in this position will report to the Nursing Supervisor (RN) and will receive clinical consultation support from the Chief Physician. Associate’s Degree Required. NYS LPN Licensed in NYS is required. One (1) year of experience working with children and adolescents involved in child welfare. Ability to facilitate training or coaching around medical conditions and treatment.
