Assist the nurse, psychologist, and residential leadership in coordinating and attending medical appointments for individuals, ensuring follow-up care and accurate documentation in Precision Care. Act as a liaison between healthcare providers, staff, and management while advocating for the best possible care. Support with scheduling, medication supply, and required paperwork under the direction of the nurse or supervisor.

Requirements: HS diploma/GED required. Valid driver’s license required. 1-3 years of experience working with the developmentally disabled is preferred. Effective communication skills, knowledge of basic medical terminology, good organizational and time management skills, and basic computer are also preferred. Those who do not have the trainings such as First Aid, CPR, AMAP, and SCIP will be required to pass and complete all within the first three months of employment.
